Since you have salary income ( primary source) and commission income (profession), ,you may decide to show Commission income either as income from other sources or as professional income.
( Note, ITR 3(presumptive) is only available to specified professions and commission income does not belong to list)

If you wish to offer commission income as other sources (recommended), file ITR-1, show your salary income under salary column and the LIC commission income under “income from other sources”.

Declare savings and figure tax liability on total income

Take TDS credit of any TDS deducted on commission income as also salary income

Follow normal steps for filing return

If you wish to offer commission income as profession , file ITR 4
For salary portion , fill salary schedule.
Go to the page “Nature of Business” and select the appropriate code for your business ( like if you are getting commission for acting as agent, select code 0301- general commission agents).

Now, come to “Part A-BS” page. If you maintain a proper balance sheet, fill in entire details in this page. If you don't maintain a proper balance sheet, then go down to the bottom of that page and fill in the last four columns only i.e. amount of sundry debtors, creditor, stock and cash. If you don't have debtors, creditors ,stock then fill them with zero.

Go to the “profit and loss” page. similarly if you maintain a profit and loss account fill in the details of profit and loss account and if you don't maintain profit and loss account then go to the bottom of that page and fill only the last four details wherein you would show the amount of gross commission in the column of gross receipts.
